Privacy Practices

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

Our duties

We are required by law to protect the privacy of your protected health information, to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect. If we change this notice, the new terms will apply to all information we hold, and the revised notice will be posted here and in our office.

How we may use and disclose your health information

For treatment. Your doctors and clinical staff use your information to provide and coordinate your care, and may share it with other providers involved in your treatment, such as a laboratory or a specialist we refer you to.

For payment. To bill and collect for services, and, if you ask us to, to provide documentation you may submit to your own insurer.

For health care operations. Quality review, training, and running the practice.

Appointment reminders and follow-up. We may contact you to remind you of an appointment or to follow up on your care, by phone, text, email, or mail, using the contact details you gave us.

Where the law requires or permits. Including public health reporting, suspected abuse or neglect, health oversight, court orders and subpoenas, law enforcement in defined circumstances, coroners and funeral directors, organ donation, approved research, serious threats to health or safety, workers’ compensation, and military or national security purposes.

Uses that always require your written authorisation

• Most uses and disclosures of psychotherapy notes
• Use or disclosure for marketing
• Any sale of your protected health information
• Most other uses not described in this notice

You may revoke an authorisation in writing at any time, except where we have already acted on it.

Your rights

• To inspect and copy your health information, in paper or electronic form. We may charge a reasonable, cost-based fee.
• To ask us to correct information you believe is wrong or incomplete. We may decline, and will tell you why in writing.
• To an accounting of certain disclosures we have made.
• To request restrictions on how we use or share your information. We are not required to agree, except that if you pay in full out of pocket for a service, you may direct us not to share information about it with a health plan, and we must honour that.
• To request confidential communication at an alternative address or telephone number.
• To a paper copy of this notice, even if you agreed to receive it electronically.
• To be notified if a breach occurs that compromises the privacy or security of your information.

Complaints

If you believe your privacy rights have been violated, contact our Privacy Officer using the details below. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
